Deliver essential nursing care in a field-based home health role focused on fostering patient well-being in their own homes. This full-time, 13-week contract opportunity is ideal for experienced Registered Nurses passionate about providing personalized, high-quality care outside the traditional clinical setting.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM

    • Role Highlights:

  • Caseload expectation: 30 points per week (typically 3–5 patient visits daily)
  • Daily travel to patient homes within an assigned territory (mileage reimbursement provided)
  • Utilize Home Care Home Base (HCHB) for all electronic documentation
  • OASIS assessment tool for patient evaluation and plan of care development

    • What You’ll Do:

  • Conduct thorough OASIS assessments and develop detailed care plans tailored to individual patient needs
  • Deliver skilled nursing services: wound care, medication administration, IV therapy, and chronic disease management
  • Monitor patient progress, note any changes, and collaborate with physicians and the broader care team
  • Provide patient and family education on disease processes, medication management, and health maintenance
  • Ensure accurate, timely documentation in HCHB
  • Coordinate care with physical therapists, occupational therapists, social workers, and home health aides
  • Participate in interdisciplinary case conferences, staff meetings, and ongoing professional development
  • Maintain compliance with regulatory guidelines and agency standards

    • Requirements:

  • Current and active RN license (in good standing) in the state of practice
  • A minimum of 1 year of recent home health experience (field experience highly preferred)
  • Proficiency with Home Care Home Base (HCHB) and OASIS documentation
  • Reliable transportation, valid driver’s license, and active auto insurance

    $1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
