Deliver compassionate, patient-centered care in a field-based home health role supporting individuals across Sumpter, OR. This full-time, 13-week contract is an ideal fit for experienced Registered Nurses ready to make an immediate impact while enjoying autonomy and meaningful patient connections.

Schedule

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Expect a caseload averaging 3–5 patient visits daily (30 points per week)

    • Key Responsibilities

  • Perform thorough OASIS assessments to develop personalized care plans for each patient.
  • Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management in the comfort of patients’ homes.
  • Monitor patient progress, identify changes in condition, and communicate updates to physicians and the interdisciplinary care team promptly.
  • Educate patients and their families about diagnoses, care regimens, medications, and preventive strategies.
  • Maintain detailed, prompt electronic documentation using Home Care Home Base (HCHB).
  • Collaborate closely with physical and occupational therapists, social workers, and home health aides for cohesive care delivery.
  • Participate in case conferences, staff meetings, and ongoing professional development.
  • Comply with all regulatory and organizational standards for care and documentation.

    • Qualifications

  • Active, unencumbered RN license in the state of Oregon
  • At least one year of recent home health nursing experience (field-based preferred)
  • OASIS assessment and Home Care Home Base (HCHB) proficiency required
  • Reliable transportation, valid driver’s license, and current auto insurance

    $2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
