Schedule/Hours:

  • Contract assignment; schedule discussed during interview.
  • Required Qualifications:

  • Active LPN license in good standing
  • At least one year of recent home health experience
  • Proficiency with HomeCare HomeBase (HCHB) EMR
  • Strong clinical assessment and documentation skills
  • Reliable transportation for regular home visits
  • Ability to work independently while coordinating across care teams
  • Key Responsibilities:

  • Deliver skilled nursing care in patient homes, following individualized care plans
  • Monitor and document each patient’s progress, updating care plans as needed
  • Administer medications and carry out treatments per state and regulatory guidelines
  • Educate patients and families about disease prevention and ongoing care management
  • Maintain accurate records using the HomeCare HomeBase (HCHB) EMR platform
  • Collaborate effectively with RNs, therapists, and other healthcare professionals to ensure holistic patient care
