Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Occasional flexibility for patient needs
  • Key Qualifications:

  • Active and unrestricted LPN license in the state of practice
  • At least 1 year of experience in home health, hospice, or similar community-based care
  • Proficiency with the Home Care Home Base (HCHB) charting system
  • Current BLS certification
  • Valid driver’s license, reliable vehicle, and insurance
  • Excellent clinical judgment, organizational, and communication skills
  • Ability to work independently and prioritize care in a field environment
  • Preferred Experience:

  • Background in Medicare-certified home health agencies
  • Knowledge of home health compliance and documentation standards
  • Patient-centered, compassionate approach in all interactions
  • Primary Responsibilities:

  • Conduct skilled nursing visits, assess patients, and implement individualized care plans
  • Manage medications, provide wound care, and oversee clinical interventions as prescribed
  • Monitor and document changes in patient conditions, reporting findings to the RN Case Manager or physician
  • Educate patients and caregivers on disease management, medication, and self-care
  • Accurately document all care and communications using HCHB software
  • Maintain infection control and patient safety throughout each visit
  • Participate in interdisciplinary team meetings and ongoing training
