Elevate your nursing career by providing essential care to patients in their homes as a Home Health Licensed Practical Nurse (LPN). In this contract role, you'll play a vital part in supporting patient independence, education, and recovery, ensuring high-quality nursing care in a field-based setting. Your expertise will help patients thrive while you work collaboratively with families and the interdisciplinary team.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Occasional flexibility to meet patient needs
  • Key Responsibilities:

  • Conduct skilled nursing visits, assessing patient needs and implementing personalized care plans.
  • Administer medications, provide wound care, and manage IV therapies or other clinical interventions as ordered.
  • Monitor patient conditions, promptly reporting significant findings to the RN Case Manager or physician.
  • Educate patients and caregivers on disease processes, medication management, and self-care techniques.
  • Maintain precise and timely documentation using Home Care Home Base (HCHB).
  • Collaborate closely with the care team to coordinate and ensure compliance with treatment plans.
  • Uphold infection control protocols and patient safety during each visit.
  • Participate in case conferences, quality improvement initiatives, and mandatory training.
  • Qualifications:

  • Active, unrestricted LPN license in the state of practice.
  • Minimum one year of home health, hospice, or similar community-based nursing experience.
  • Proficiency with Home Care Home Base (HCHB) is required.
  • Valid driver’s license, reliable transportation, and current insurance.
  • BLS (Basic Life Support) certification.
  • Skilled in clinical judgment, time management, and communication.
  • Ability to work independently in the field. Preferred:
  • Experience with Medicare-certified home health agencies.
  • Deep understanding of home health compliance and documentation.
  • Exceptional interpersonal and patient-focused care skills.
