As a Home Health Licensed Practical Nurse (LPN), you will work as a field nurse delivering individualized nursing care, helping patients achieve greater independence, and supporting their recovery journeys. This full-time contract opportunity involves managing a caseload of home-based patients Monday through Friday, 8:00 AM – 5:00 PM, with occasional flexibility to accommodate patient needs. Expect to complete 3–5 visits per day (equivalent to 25–32 points weekly), utilizing Home Care Home Base (HCHB) for seamless and accurate documentation.

$1,304 - $1,380/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
