An experienced and compassionate nursing professional is needed to provide high-quality care to patients in their homes. This contract opportunity involves working with a dedicated interdisciplinary team to deliver patient-centered services and ensure positive outcomes in a field-based setting.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • 13-week contract duration

    • Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized care plans for each patient.
  • Deliver skilled nursing care including wound care, medication administration, IV therapy, and management of chronic illnesses.
  • Continuously evaluate patient progress, recognize changes in condition, and coordinate promptly with physicians and team members.
  • Educate patients and families about disease processes, medication management, treatment options, and preventive care.
  • Maintain accurate, timely electronic documentation using Home Care Home Base (HCHB).
  • Work collaboratively with therapists, social workers, and aides to ensure effective, coordinated care delivery.
  • Attend case conferences, staff meetings, and engage in ongoing professional development.
  • Follow all regulatory and agency guidelines for documentation and care standards.
  • Daily travel to patient homes within an assigned territory (mileage reimbursement provided).

    • Qualifications:

  • Active RN license in good standing
  • At least 1 year of recent field-based home health experience required
  • Proficiency in HCHB and OASIS is essential
  • Strong assessment, critical thinking, organizational, and communication skills
  • Reliable transportation, valid driver’s license, and auto insurance
  • Ability to operate independently and manage a flexible, mobile schedule

    • Preferred:

  • BSN degree
  • Experience managing a caseload of 25–32 points per week
  • Familiarity with Medicare and compliance standards for home health

    $2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
