Deliver high-quality, patient-centered care where it matters most—as a Home Health Registered Nurse (RN). This contract opportunity focuses on providing compassionate, skilled nursing to individuals in their homes, ensuring continuity and excellence in patient care.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Full-time, 13-week contract
  • Field-based visits within assigned territory, with daily travel (mileage reimbursement provided)
  • Caseload expectation: 25–32 points per week (typically 3–5 patient visits daily)
  • Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop care plans tailored to each patient’s needs
  • Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management
  • Monitor patient progress, promptly communicate changes, and coordinate with care teams and physicians
  • Educate patients and families on diagnoses, treatments, and preventive care to support optimal outcomes
  • Maintain timely, accurate documentation using Home Care Home Base (HCHB)
  • Collaborate with interdisciplinary teammates—physical therapists, occupational therapists, social workers, and home health aides
  • Participate in meetings, case conferences, and ongoing educational initiatives
  • Ensure compliance with clinical, regulatory, and documentation standards
  • Qualifications:

  • Active RN license (in good standing)
  • At least 1 year of recent Home Health experience (field experience preferred)
  • Proficient with Home Care Home Base (HCHB) and OASIS assessment tools
  • Strong clinical assessment, time management, and critical thinking skills
  • Reliable transportation, valid driver’s license, and current auto insurance
  • Ability to work autonomously with excellent communication and organization
  • BSN and prior experience managing 25–32 points per week are pluses
  • Knowledge of Medicare and home health compliance considered advantageous
