An opportunity is available for a Home Health Registered Nurse (RN) to deliver skilled, compassionate, and patient-focused care to individuals in their homes. This contract position offers the chance to positively impact patient outcomes by providing nursing expertise and fostering independence and comfort within the community.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Key Responsibilities:

  • Conduct OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing interventions, including wound care, IV therapy, chronic disease management, and medication administration.
  • Monitor patient progress, identify potential changes in condition, and coordinate promptly with the healthcare team and physicians.
  • Educate patients and families about disease processes, medications, treatments, and preventative health.
  • Accurately document all care and communications using Home Care Home Base (HCHB).
  • Collaborate with the interdisciplinary team—including physical therapists, occupational therapists, social workers, and home health aides—for coordinated case management.
  • Participate in staff meetings, case conferences, and ongoing education.
  • Ensure all care delivered complies with agency and regulatory standards.
  • Qualifications:

  • Active and unencumbered RN license in Georgia.
  • At least 1 year of recent Home Health experience; field experience highly valued.
  • Proficiency with Home Care Home Base (HCHB) electronic documentation.
  • Experience performing OASIS assessments.
  • Exceptional assessment, time management, and organizational skills.
  • Reliable transportation, valid driver’s license, and auto insurance required.
  • Ability to work independently in a field-based environment.
  • Preferred:

  • Bachelor’s degree in Nursing (BSN).
  • Comfort with a weekly caseload of 25–32 points.
  • Knowledge of Medicare and home health compliance.
