Deliver compassionate and skilled nursing care to patients in their homes as a Home Health Licensed Practical Nurse (LPN). In this essential field-based role, you will help promote patient independence and recovery by managing a dynamic caseload and supporting individuals through direct care, education, and strong collaboration with healthcare team members.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Occasional flexibility to accommodate patient needs

    • Key Responsibilities:

  • Perform skilled nursing visits, including assessment, planning, and implementation of individualized care plans for home health patients
  • Administer medications, perform wound care, and oversee IV therapy or other clinical interventions as prescribed
  • Monitor and promptly report any changes in patient condition to the RN Case Manager or physician
  • Educate patients and caregivers on disease management, medication administration, and self-care techniques
  • Accurately document all patient assessments, care, and interactions using Home Care Home Base (HCHB)
  • Comply with infection control procedures and promote patient safety at every visit
  • Collaborate with the interdisciplinary team to ensure care plan compliance and successful patient outcomes
  • Participate in case conferences, quality improvement efforts, and ongoing training

    • Required Qualifications:

  • Active, unrestricted LPN license in the state
  • At least 1 year of home health, hospice, or relevant community-based nursing experience
  • Proficient in Home Care Home Base (HCHB) documentation
  • Valid driver’s license, reliable vehicle, and current insurance
  • Current BLS certification
  • Strong clinical assessment, organizational, and communication skills
  • Ability to independently manage a field-based caseload

    • Preferred:

  • Experience with Medicare-certified home health agencies
  • In-depth knowledge of compliance and documentation requirements
  • Patient-focused approach and effective interpersonal skills

    $1,309 - $1,384/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
