Deliver compassionate, skilled nursing care in a patient’s home as an experienced Licensed Practical Nurse (LPN) focused on home health services. This contract role provides the opportunity to help individuals recover and remain independent while working in a field-based environment.

Schedule and Hours:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Occasional flexibility for patient needs
  • Approximately 25–32 points per week (3–5 home visits daily)

    • Responsibilities:

  • Perform skilled nursing visits, conducting comprehensive assessments and developing individualized care plans for patients in their homes
  • Administer medications, carry out wound care, and provide clinical interventions such as IV therapy according to provider orders
  • Monitor for changes in patients’ health status, reporting significant findings to the RN Case Manager or physician as appropriate
  • Educate patients and their families on disease processes, medication management, and self-care to support recovery and independence
  • Complete timely and accurate documentation in Home Care Home Base (HCHB) and ensure compliance with regulatory standards
  • Collaborate with the wider home health team, participating in case conferences, training, and ongoing quality improvement
  • Adhere consistently to infection control practices and promote overall patient safety

    • Qualifications:

  • Active and unrestricted LPN license in the state of Nevada
  • At least one year of recent home health, hospice, or comparable community-based experience required
  • Proficient with the Home Care Home Base (HCHB) electronic charting system
  • Valid driver’s license, reliable transportation, and current insurance
  • Current BLS certification
  • Demonstrated ability to manage caseloads independently with strong clinical judgment, time management, and communication skills
  • Previous experience with Medicare-certified agencies and solid understanding of home health documentation standards preferred

    $1,609 - $1,702/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
