In this per diem opportunity, you will conduct home health risk assessments for Medicare Advantage plan members across Creek County, OK, including areas like Sapulpa, Bristow, and Drumright. Each assessment typically takes about 45 minutes and addresses a range of needs, from annual evaluations to post-discharge visits.

Schedule:

  • Flexible shifts offered Monday through Sunday

  • Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage members
  • Review prior and current medical history, conditions, and medication regimens
  • Offer clear patient education about disease management and medication adherence
  • Identify care gaps, provide recommendations to enhance patient outcomes
  • No need to create treatment plans or prescribe medications
  • Qualifications:

  • Unrestricted Nurse Practitioner License in Oklahoma
  • Valid and current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Completion of an MSN from an accredited institution
  • Minimum of 6 months’ current NP experience preferred
  • In return for your expertise, you’ll receive $80-$90 per completed assessment, a mileage stipend, and compensation for administrative duties.
