In this per diem opportunity, you’ll perform home health risk assessments for Medicare Advantage plan members across Minidoka County, providing a vital service to the Rupert, Heyburn, and Paul communities.

  • Meet minimum weekly or bi-weekly hourly commitment
  • Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments
  • Review and evaluate medical, medication, and surgical history
  • Visit patients in their homes; average evaluation is approximately 45 minutes
  • Educate members and their families on health conditions and proper medication usage
  • Identify care gaps and recommend appropriate actions for their resolution
  • No need to develop treatment plans or prescribe medication
  • Qualifications:

  • Active Nurse Practitioner License in Idaho
  • Board Certification through AANP or ANCC
  • Current CPR/BLS certification required
  • MSN from an accredited nursing program
  • At least six months of recent Nurse Practitioner experience preferred
