In this per diem role, you will visit Medicare Advantage plan members throughout Peoria County, conducting thorough risk assessments in the comfort of the patients’ homes. Typical visits last approximately 45 minutes and may involve annual health assessments or post-discharge evaluations. This position offers unparalleled flexibility, allowing you to create your own schedule for shifts between Monday and Sunday, with either weekly or bi-weekly minimum commitment.

Schedule:

  • Minimum weekly or bi-weekly hourly commitment
  • Primary Responsibilities:

  • Perform annual risk assessments for Medicare Advantage members in patient homes throughout Peoria County
  • Assess patients’ medical history, existing conditions, and medication usage
  • Educate patients about their medical conditions and prescribed medications
  • Identify gaps in care and deliver recommendations to address these
  • No requirement to create treatment plans or prescribe medications
  • Qualifications:

  • Nurse Practitioner License valid in Illinois
  • Board Certification through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • Current Basic Life Support (BLS) certification (CPR)
  • At least 6 months of recent experience as a Nurse Practitioner preferred
  • This position offers a competitive rate of $80–$90 per evaluation, complemented by a mileage stipend and compensation for administrative time.
