  • Key Responsibilities:

  • Perform comprehensive home health risk assessments on Medicare Advantage plan members residing in areas such as Marianna, Moro, and Rondo
  • Review and evaluate medical histories, current medical conditions, and medications
  • Educate patients about medical conditions, health maintenance, and medications
  • Identify gaps in care, providing actionable recommendations for improvement
  • No requirement for developing treatment plans or prescribing medications
  • Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • Master of Science in Nursing (MSN) from an accredited institution
  • Minimum 6 months of recent NP experience preferred
