In this dynamic nurse practitioner role, you will support Medicare Advantage plan members by providing crucial health checks in various communities. This position offers flexible scheduling options, allowing you to create your own work calendar with opportunities for shifts between Monday and Sunday, and a minimum weekly or bi-weekly hourly commitment.

Schedule:

  • Self-scheduling to fit your needs
  • Minimal required commitment per week or every two weeks
  • Responsibilities:

  • Complete comprehensive, standardized home health risk assessments in patient homes
  • Review and evaluate patients’ medical histories, conditions, and medications
  • Offer education and guidance on illnesses and medical therapies
  • Identify gaps in care and suggest actionable steps for resolution
  • Document findings appropriately—no treatment planning or prescribing required
  • Qualifications:

  • Active and unrestricted Nurse Practitioner license in Virginia
  • Board certification (AANP or ANCC)
  • Current CPR certification (BLS)
  • MSN degree from an accredited program
  • At least 6 months of recent NP experience preferred
  • Competitive compensation is provided at $80–$90 per completed evaluation, in addition to a mileage stipend and compensation for administrative time.
