Advance your nursing career by supporting patients directly in their homes. As a valued Nurse Practitioner, you will perform home health risk assessments for Medicare Advantage plan members across Wapello County, IA. Your clinical expertise will ensure that each patient receives thorough, personalized evaluations during annual assessments, post-discharge visits, or as otherwise needed. Each assessment generally takes around 45 minutes, allowing you to focus on high-quality, one-on-one care.

Schedule:

  • Flexible shifts offered seven days a week
  • Self-scheduling: adapt your hours to fit your needs
  • Choose your weekly or bi-weekly commitment level
  • Primary Responsibilities:

  • Visit patients in Ottumwa, Eldon, Agency, and other local communities to conduct comprehensive home health risk assessments
  • Review and evaluate patients’ medical histories, current conditions, and medication usage
  • Provide essential patient education regarding disease management and safe medication use
  • Identify care gaps, offering recommendations to close them and optimize patient outcomes
  • Note: This role does not require prescribing medication or establishing treatment plans
  • Desired Experience and Qualifications:

  • Active Nurse Practitioner license in Iowa
  • MSN from an accredited nursing program
  • Board Certification via AANP or ANCC
  • Basic Life Support (BLS) CPR certification up-to-date
  • Preferred: At least 6 months of current nurse practitioner experience, particularly in home health or related fields
