Deliver vital care in a flexible, patient-centered setting by performing home health risk assessments for Medicare Advantage plan members. This per diem opportunity allows you to create your own schedule, providing essential assessments and education to patients in various Jefferson County communities, such as Arnold, Imperial, Festus, and Barnhart.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Self-scheduling with minimum weekly or bi-weekly hourly commitment
  • Key Responsibilities:

  • Conduct comprehensive home health risk assessments (approximately 45 minutes each) in patient homes
  • Evaluate prior and current medical history, existing conditions, and medication regimens
  • Educate patients on managing their health conditions and understanding medications
  • Identify gaps in care and offer actionable recommendations to help close those gaps
  • Visits may include annual health assessments or post-discharge check-ins
  • No requirement to create treatment plans or prescribe medications
  • Qualifications:

  • Active Nurse Practitioner license in Missouri
  • Board Certification from AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • Current CPR/BLS certification
  • At least six months of experience as a Nurse Practitioner preferred
  • Home health experience is an asset
  • As part of this role, you’ll earn between $80–$90 per evaluation, with an additional mileage stipend and compensation for administrative time.
