Deliver quality care with autonomy in a dynamic home health environment by joining a specialized team of Nurse Practitioners. This per diem opportunity involves conducting risk assessments for Medicare Advantage members in Gadsden County, including Quincy, Chattahoochee, and Havana. Each visit, lasting roughly 45 minutes, may involve annual assessments or post-discharge evaluations—providing essential guidance without the need for medication prescribing or treatment plan development.

  • Minimum weekly or bi-weekly hourly commitment required
  • Key Responsibilities:

  • Conduct comprehensive annual risk assessments for Medicare Advantage plan members
  • Review prior and current medical history, conditions, and medications
  • Deliver patient education to support health and medication understanding
  • Identify gaps in care, offering informed recommendations for improvement
  • Document assessments and findings in accordance with organizational standards
  • Qualifications:

  • Active Nurse Practitioner license in Florida (FL)
  • Current BLS (CPR) certification
  • Board certification via AANP or ANCC
  • Minimum 6 months’ experience as a practicing NP preferred
  • MSN degree from an accredited nursing program
  • As part of this role, you’ll earn $80–$90 per evaluation, with additional mileage and administrative stipends included, allowing you to be compensated for both your clinical expertise and the time dedicated to travel and reporting.
