As part of this role, you will conduct risk assessments for Medicare Advantage plan members in various locations throughout Lake County, FL, including Clermont, Leesburg, and Eustis. Assessments typically take about 45 minutes and may include annual health evaluations or post-discharge visits for plan members in their homes.
