Deliver comprehensive patient care in a flexible, per diem role focused on home health risk assessments for Medicare Advantage plan members in Merrimack County. As an advanced practice clinician, you will visit patients in their homes—covering areas including Concord, Hooksett, and Bow—to conduct thorough annual or post-discharge assessments. Each evaluation generally lasts about 45 minutes and is designed to help bridge gaps in care while enhancing patient health education.

Schedule:

  • Flexible shifts available Monday through Sunday

  • Minimum weekly or bi-weekly hourly commitment required
  • Key Responsibilities:

  • Perform annual risk assessments for a Medicare Advantage health plan
  • Review and evaluate prior/current medical history, health conditions, and medications
  • Educate patients about their medical conditions and prescriptions
  • Identify care gaps and offer actionable recommendations to resolve them
  • No treatment plan development or medication prescribing required
  • Qualifications:

  • Active Nurse Practitioner license in New Hampshire
  • Board Certification via AANP or ANCC
  • MSN degree from an accredited nursing school
  • Current BLS (CPR) certification
  • At least 6 months of recent experience working as a Nurse Practitioner is preferred
