Schedule

  • Key Responsibilities

  • Perform comprehensive home health risk assessments, each lasting approximately 45 minutes, for Medicare Advantage members
  • Review and evaluate both prior and current medical histories, existing conditions, and medication regimens
  • Offer patient education regarding chronic conditions and prescribed medications
  • Identify gaps in patient care and recommend actionable solutions
  • Visits may include annual reviews or post-discharge assessments
  • No treatment plan creation or medication prescribing is required
  • Qualifications

  • Active Nurse Practitioner License in Ohio
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Preferred: At least 6 months of Nurse Practitioner experience
  • MSN degree from an accredited nursing program
  • This opportunity offers compensation of $80–$90 per completed evaluation, plus a mileage stipend and payment for administrative time. You will primarily visit members in Norwalk, Willard, Bellevue, and surrounding areas within Huron County.
