Take your advanced clinical skills into the community as a Nurse Practitioner expert in home health assessments. This per diem opportunity is ideal for practitioners seeking a flexible schedule and meaningful, independent patient engagement. You’ll conduct in-depth risk assessments for Medicare Advantage plan members living in Des Moines, Cedar Rapids, Davenport, and surrounding communities across Des Moines County, IA. Each assessment takes approximately 45 minutes, with visit types including both comprehensive annual health reviews and post-discharge check-ins—no ongoing treatment plans or medication prescribing required.

Schedule:

  • Flexible shifts available any day, Monday through Sunday
  • Self-determined schedule tailored to your availability
  • Minimum hourly commitment set weekly or bi-weekly
  • Core Responsibilities:

  • Perform thorough annual home health risk assessments for Medicare Advantage members.
  • Review and evaluate prior/current medical history, chronic conditions, and medication regimens.
  • Offer patient-focused education regarding health conditions and medication use.
  • Detect gaps in care and deliver actionable recommendations for improvement.
  • Focus strictly on risk assessment and guidance—treatment planning and prescribing are not part of this position.
  • Qualifications:

  • Active Nurse Practitioner license in Iowa
  • Current BLS/CPR certification
  • Board Certification via AANP or ANCC
  • Prefer six months or more of Nurse Practitioner experience
  • Master of Science in Nursing from an accredited institution
