Take on a dynamic per diem role supporting Medicare Advantage members with comprehensive, in-home health risk assessments in Jerome County, ID. As a Nurse Practitioner working in the home health setting, you’ll conduct focused evaluations that play a critical role in shaping ongoing care. This opportunity features autonomy in scheduling and the ability to make a direct impact in multiple communities, including Jerome, Hazelton, Eden, and surrounding areas.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling for optimal work-life balance
  • Minimum weekly or bi-weekly hourly commitments
  • Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage plan populations
  • Review and evaluate past and current medical history, diagnoses, and medication regimens
  • Deliver patient education concerning health conditions and prescribed treatments
  • Identify care gaps and provide actionable recommendations for improved outcomes
  • No need to create ongoing treatment plans or prescribe medications—focus is on assessment
  • Qualifications:

  • Active, unrestricted Nurse Practitioner license in Idaho
  • Current CPR certification (BLS)
  • National Board Certification (AANP or ANCC)
  • Minimum MSN from an accredited nursing program
  • At least six months of recent Nurse Practitioner experience preferred
  • This position provides $80–$90 per completed evaluation, in addition to a mileage stipend and compensation for administrative time.
