Make a meaningful impact delivering high-quality care to patients in their homes by conducting comprehensive health assessments. As a Nurse Practitioner with a home health focus, you will perform risk evaluations on Medicare Advantage plan members throughout Onslow County—including Jacksonville, Holly Ridge, and Swansboro. The nature of each assessment may include annual visits or post-discharge evaluations, tailored to the patients’ needs.

  • Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage members in their residences
  • Gather detailed histories, including prior and current medical conditions and medication regimens
  • Educate patients on health conditions, preventive care, and correct medication usage
  • Identify gaps in care and provide targeted recommendations to enhance patient outcomes
  • No requirement for creating treatment plans or prescribing medications
  • Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Current Basic Life Support (BLS/CPR) certification
  • Board certification from the American Association of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC)
  • Completion of a Master of Science in Nursing from an accredited program
  • Preferred: at least 6 months of current Nurse Practitioner experience, ideally in home health
  • This per diem opportunity offers compensation of $80-$90 per evaluation, with a mileage stipend and compensation for administrative time, ensuring your dedication and expertise are recognized.
