Take your Nurse Practitioner expertise directly into the community as a valued provider performing home health risk assessments for Medicare Advantage plan members throughout Adams County. This per diem opportunity offers a flexible schedule—choose shifts between Monday and Sunday with the ability to set your own hours and weekly or bi-weekly commitments. Each home visit, generally lasting about 45 minutes, will focus on comprehensive health evaluations in varied environments across Quincy, Camp Point, Payson, and surrounding areas.

  • Set your preferred minimum weekly or bi-weekly hourly commitments
  • Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage members in their homes
  • Review and evaluate patients’ current and prior medical history, health conditions, and medication use
  • Provide patient-focused education on chronic conditions and prescriptions
  • Identify gaps in care and recommend next steps to support optimal health
  • No requirement to prescribe medications or develop treatment plans
  • Qualifications:

  • Active Nurse Practitioner license in Illinois
  • Current CPR certification (BLS)
  • Board certified through AANP or ANCC
  • Minimum of 6 months’ recent experience as a Nurse Practitioner preferred
  • MSN degree from an accredited nursing program
