Deliver high-quality, patient-centered care in a per diem role as a Nurse Practitioner specializing in home health risk assessments. This opportunity involves conducting comprehensive health evaluations for Medicare Advantage plan members residing throughout Knox County, ME, including Rockland, Camden, and nearby areas. Each assessment typically lasts about 45 minutes and may be scheduled as either an annual health review or as a post-discharge visit.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Ability to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment
  • Key Responsibilities:

  • Perform annual home health risk assessments for members of a Medicare Advantage health plan
  • Review patients’ prior and current medical histories, along with their medication profiles and ongoing conditions
  • Educate patients regarding their medical conditions and medication regimens
  • Identify any existing gaps in care and make recommendations to address these gaps
  • No prescribing of medications or development of treatment plans required
  • Qualifications:

  • Valid Nurse Practitioner license in ME
  • Current CPR certification (BLS)
  • Board certification via AANP or ANCC
  • A minimum of six months of NP experience (preferred)
  • MSN from an accredited nursing school
