Bring your nursing expertise directly to patients as a Nurse Practitioner providing home health risk assessments within Lawrence County. This per diem opportunity offers the flexibility to set your own schedule, with shifts available throughout the week, including weekends. You'll complete thorough assessments in patients’ homes, supporting Medicare Advantage plan members in Ironton, South Point, Proctorville, and surrounding areas. Each visit averages about 45 minutes and may include annual health assessments or post-discharge evaluations.

Schedule & Flexibility:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule to meet individual needs
  • Commitment can be minimum weekly or bi-weekly, based on your availability
  • Primary Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage members
  • Review prior and current medical histories, medical conditions, and medication use
  • Educate patients about their conditions and prescribed medications
  • Identify care gaps and offer recommendations to enhance patient outcomes
  • No requirement to develop treatment plans or prescribe medications
  • Qualifications:

  • Active Nurse Practitioner License in the State of Ohio
  • Board Certification through AANP or ANCC
  • Current BLS certification (CPR)
  • Master of Science in Nursing (MSN) from an accredited program
  • Minimum of 6 months recent Nurse Practitioner experience preferred
  • Compensation & Perks: In this role, you will earn $80–$90 per completed evaluation, plus receive mileage reimbursement and compensation for administrative time.
