Schedule:

  • Rotating weekends required
  • Coverage needed Sunday through Saturday, hours may vary
  • Qualifications and Desired Experience:

  • Current, active RN license in good standing
  • Minimum of one year experience in rehabilitation or long-term care nursing
  • Strong assessment, documentation, and communication skills
  • Ability to collaborate with interdisciplinary teams across therapy, social work, and case management
  • Basic life support (BLS) certification
  • Responsibilities:

  • Assess, plan, implement, and evaluate individualized care for patients undergoing rehabilitation or residing in long-term care
  • Administer medications and treatments per physician orders and regulatory standards
  • Provide support and education to patients and their families about ongoing care needs and progress
  • Monitor and record patient outcomes, updating care plans as necessary
  • Ensure a clean, safe, and dignified environment for all residents
  • Respond promptly and effectively to any patient emergencies
