Deliver high-quality nursing care and make a meaningful impact in patients' lives by providing skilled services within a supportive home health environment. This full-time, 13-week contract opportunity is perfect for experienced Registered Nurses ready to achieve excellence in a field-based role.

Schedule & Setting:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Field-based: visit patients in their homes within a defined territory
  • Expect 30 points per week (3–5 patient visits per day)

    • Key Responsibilities:

  • Conduct thorough OASIS assessments and develop personalized care plans for each patient
  • Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management
  • Monitor patient conditions, recognize changes, and collaborate closely with the care team and physicians
  • Provide education to patients and their families about diagnoses, treatment regimens, medications, and preventive strategies
  • Complete documentation promptly and accurately in Home Care Home Base (HCHB)
  • Work cooperatively with physical therapists, occupational therapists, social workers, and home health aides to coordinate comprehensive care
  • Participate in case conferences, staff meetings, and ongoing education as requested
  • Uphold all regulatory and agency standards in documentation and patient care delivery

    • Qualifications:

  • Active RN license (state of practice), in good standing
  • At least 1 year of recent home health nursing experience, with field-based experience strongly preferred
  • Hands-on proficiency with Home Care Home Base (HCHB) and the OASIS assessment tool
  • Reliable transportation, current driver's license, and auto insurance

    $2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
