This contract position is ideal for a dedicated Registered Nurse eager to make a tangible difference by assessing conditions, implementing individualized care plans, and providing critical education and support to patients and families.

Schedule:

  • Weekend rotations required once every 6 weeks

    • Essential Qualifications:

  • Active RN license in good standing
  • Minimum of 1 year of certified home care experience
  • At least 1 year of case management experience
  • Reliable transportation and ability to travel to assigned patient cases throughout the community
  • Strong assessment, critical thinking, and organizational skills
  • Proficiency with electronic health records and interdisciplinary communication
  • Committed to upholding established standards and policies for community health care

    • Key Responsibilities:

  • Conduct thorough patient assessments and craft individualized care plans
  • Manage ongoing case responsibilities, including admission processes and follow-up care
  • Educate patients and families regarding conditions, treatments, and post-care requirements
  • Document patient encounters and care plan updates in a timely and comprehensive electronic format
  • Collaborate professionally with interdisciplinary care teams to achieve optimal patient outcomes
  • Adhere to all organizational policies, procedures, and standards for quality and compliance
