Schedule/Hours:

  • Flexible weekly scheduling; schedule cycle is every 13 weeks
  • No mandatory weekends or on-call, unless you choose to participate
  • Holiday coverage required for 75% of holidays within assignment
  • 6 home visits per day (routine, discharge, ROC, recert, reassessment)
  • Paid mileage reimbursement for all travel days
  • Required Qualifications & Experience:

  • Active RN license
  • At least 1 year of recent Home Health experience (seasoned for first travel assignments)
  • Proficiency in Homecare Homebase (HCHB) electronic medical record system
  • Current BLS certification
  • Valid driver’s license and proof of insurance
  • Competence in IV, pleurx, wound vac, wound care, OASIS, assessment, patient education, and comprehensive case management
  • Role Responsibilities:

  • Deliver in-home skilled nursing care to a variety of patients with chronic and complex needs
  • Complete all documentation via provided Samsung Note 9 phones
  • Schedule patient visits a day in advance, maintaining weekly visit expectations
  • Collaborate with departmental leaders, schedulers, and clinical peers
  • Travel between assigned communities, optimize care plans, and educate patients and families

    $1,815 - $1,920/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
