An opportunity is available for a talented and compassionate Home Health Registered Nurse to deliver high-quality care to patients in a home setting. This contract position offers a structured weekday schedule and a focus on patient-centered care in partnership with a dedicated healthcare team.

Schedule:

  • Full-time, 13-week contract
  • Monday – Friday, 8:00 AM – 5:00 PM

    • Responsibilities:

  • Complete comprehensive OASIS assessments and develop individualized plans of care.
  • Deliver direct nursing services, including wound care, IV therapy, medication management, and chronic disease support.
  • Closely monitor patient progress, identify care needs, and relay updates to physicians and team members.
  • Educate patients and families on health conditions, treatments, and wellness strategies.
  • Maintain up-to-date and accurate documentation in Home Care Home Base (HCHB).
  • Coordinate care with therapists, social workers, and home health aides to achieve optimal outcomes.
  • Participate in ongoing staff meetings, education, and case conferences.

    • Required Qualifications:

  • Active Registered Nurse (RN) license in good standing for the state of practice.
  • At least 1 year of recent field Home Health RN experience.
  • Proven proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
  • Capable of effectively managing a caseload of 25–32 points per week (typically 3–5 visits per day).
  • Strong clinical assessment, critical thinking, and communication abilities.
  • Reliable transportation with valid driver’s license and insurance.
  • Self-motivated, organized, and adept at working independently in the field.

    • Preferred:

  • Bachelor of Science in Nursing (BSN).
  • Familiarity with Medicare and home health compliance requirements.
  • Past experience with managing similar caseloads.

    $1,954 - $2,067/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
