Deliver exceptional care and make a meaningful impact in your community as a Home Health Registered Nurse (RN). This full-time, 13-week contract offers you the opportunity to provide high-quality, patient-centered care directly in the homes of those who rely on your clinical expertise.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM

    • Key Responsibilities:

  • Conduct comprehensive OASIS assessments to develop individualized care plans for each patient.
  • Deliver skilled nursing interventions, such as wound care, IV therapy, medication administration, and chronic disease management.
  • Monitor patient progress, identify changes in condition, and promptly update the care team and physicians.
  • Educate patients and their families on medications, disease processes, preventive strategies, and overall care management.
  • Maintain accurate and timely electronic records within Home Care Home Base (HCHB).
  • Coordinate care and communicate effectively with an interdisciplinary team, including therapists, social workers, and home health aides.
  • Participate in case conferences, staff meetings, and ongoing training.
  • Ensure compliance with agency policies and regulatory standards.

    • Required Qualifications:

  • Active RN license in good standing for the state of practice
  • Minimum 1 year of recent, hands-on Home Health RN experience (field experience highly preferred)
  • Proficiency with OASIS and Home Care Home Base (HCHB)
  • Demonstrated strong clinical, communication, and organizational skills
  • Reliable transportation, valid driver’s license, and current auto insurance
  • Independent, self-directed work ethic with excellent time management

    • Preferred:

  • BSN
  • Experience with a 25–32 weekly point caseload
  • Familiarity with Medicare/home health compliance
