Take the next step in your nursing career as a Home Health Registered Nurse, providing one-on-one skilled care in the Shepherdsville, KY area. Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • 13-week contract
  • Caseload: 25–32 points per week (typically 3–5 patient visits daily)
  • Daily local travel within your assigned territory (mileage reimbursement provided)

    • Responsibilities:

  • Complete thorough patient assessments using OASIS and develop personalized care plans
  • Deliver high-quality nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management
  • Closely monitor patient status, notifying physicians and the care team of any changes
  • Educate patients and their families regarding care plans, medications, and preventative health strategies
  • Collaborate with interdisciplinary team members for optimal patient outcomes
  • Maintain accurate and timely electronic documentation using Home Care Home Base (HCHB)
  • Participate in case conferences, team meetings, and ongoing education

    • Qualifications:

  • Current and active RN license in Kentucky (or compact state)
  • At least 1 year of recent home health experience in the field
  • Proficiency with Home Care Home Base (HCHB) and demonstrated OASIS assessment skills
  • Strong assessment, critical thinking, and organizational abilities
  • Reliable transportation, valid driver’s license, and auto insurance
  • Ability to work autonomously while managing a variable daily schedule

    • Preferred:

  • BSN degree
  • Experience maintaining a caseload of 25–32 points per week
  • Familiarity with Medicare and home health compliance guidelines
