A rewarding opportunity is available for an experienced and compassionate Registered Nurse to deliver exceptional home health care services. This field-based position involves providing high-quality, patient-centered care to individuals in their homes, supporting their health and independence. The role requires clinical excellence, strong communication, and the ability to work autonomously while managing a structured caseload.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Full-time, 13-week contract
  • Caseload: 25–32 points per week (typically 3–5 patient visits per day)

    • Key Responsibilities:

  • Conduct in-depth OASIS assessments and develop individualized care plans.
  • Provide a wide range of skilled nursing services, such as wound care, IV therapy, medication administration, and chronic disease management.
  • Monitor patient progress, promptly report condition changes, and coordinate care with interdisciplinary team members.
  • Educate patients and families about illness management, medications, and preventive strategies.
  • Accurately document all care and changes within the Home Care Home Base (HCHB) system.
  • Participate in case conferences, staff meetings, and ongoing education as needed.
  • Travel daily to patient residences within the assigned territory; mileage reimbursement is provided.

    • Qualifications:

  • Active RN license in good standing for the state of practice.
  • At least 1 year of recent home health experience, including fieldwork.
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
  • Strong clinical assessment, time management, and communication skills.
  • Reliable transportation, valid driver’s license, and auto insurance required.
  • BSN and knowledge of Medicare/home health standards are a plus.
