An excellent opportunity is available for a Home Health Registered Nurse (RN) to deliver expert care to patients in a field-based setting. This contract position focuses on providing high-quality nursing services directly in patients’ homes, with a Monday–Friday, 8:00 AM–5:00 PM schedule for a 13-week engagement. Expect a caseload of 25–32 points weekly, typically translating to three to five home visits per day. Mileage reimbursement is provided for daily travel throughout the assigned territory.

Key Responsibilities:

  • Complete comprehensive OASIS assessments and craft individualized care plans.
  • Administer skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management.
  • Continuously assess patient progress and promptly communicate changes with the interdisciplinary healthcare team.
  • Offer education to patients and caregivers regarding diagnoses, care regimens, medications, and prevention.
  • Maintain meticulous documentation using Home Care Home Base (HCHB).
  • Actively collaborate with physical therapists, occupational therapists, social workers, and aides for holistic patient care.
  • Participate in case reviews, staff meetings, and ongoing professional development.
  • Ensure compliance with all regulatory and agency-specific standards.

    • Qualifications:

  • Unencumbered, active RN license in the state of practice.
  • At least one year of recent field-based Home Health RN experience.
  • Proficient use of Home Care Home Base (HCHB) and OASIS assessment tools.
  • Reliable transportation, current driver’s license, and valid auto insurance.
  • Strong assessment, critical thinking, time management, and communication skills.
  • Ability to independently manage workflow and schedules.
  • BSN and prior experience with 25–32 points per week and Medicare/home health compliance standards are preferred.

    $2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
