An opportunity is available for an experienced and compassionate Registered Nurse to deliver high-quality care to patients in their homes. This contract position focuses on providing skilled, individualized nursing interventions while working alongside a multidisciplinary team to achieve optimal patient outcomes.

Schedule:

  • Full-time, 13-week contract
  • Monday – Friday, 8:00 AM – 5:00 PM
  • Daily travel required throughout the assigned territory (mileage reimbursement included)
  • Expected caseload: 25–32 points per week (typically 3–5 patient visits per day)
  • Field-based, using Home Care Home Base (HCHB) and OASIS assessments

    • Responsibilities:

  • Complete comprehensive OASIS assessments and develop tailored care plans
  • Deliver skilled nursing care: wound care, medication administration, IV therapy, and chronic disease management
  • Monitor patient status, promptly identify changes, and communicate effectively with physicians and care team members
  • Educate patients and their families on treatment plans and health management strategies
  • Maintain accurate, timely documentation in HCHB
  • Collaborate with other healthcare providers to coordinate interventions and manage care transitions
  • Attend case conferences, staff meetings, and participate in ongoing education
  • Comply with all agency and regulatory documentation requirements

    • Qualifications:

  • Current, active RN license in the state of practice
  • At least 1 year of recent home health experience (field experience preferred)
  • Required proficiency with HCHB and OASIS
  • Strong clinical assessment, critical thinking, and organizational skills
  • Reliable transportation, valid driver’s license, and auto insurance
  • Ability to work independently while meeting caseload expectations
  • BSN preferred; experience managing 25–32 points per week and knowledge of Medicare/home health compliance a plus

    $1,970 - $2,084/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
