Seeking an experienced and compassionate Registered Nurse to provide high-quality home health care for patients in a field-based setting. This is a 13-week contract assignment, offering an opportunity to make a meaningful impact by delivering patient-centered nursing care and supporting individuals as they recover and manage chronic conditions at home.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM

    • Key Responsibilities:

  • Conduct OASIS assessments and create individualized care plans for each patient.
  • Deliver skilled nursing, including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor and evaluate patient progress, responding to any changes in condition and communicating with the interdisciplinary care team and physicians.
  • Educate patients and their families regarding treatment plans, medication regimens, disease management, and preventive health.
  • Maintain excellent documentation of patient care using Home Care Home Base (HCHB).
  • Coordinate care and collaborate with physical therapists, occupational therapists, medical social workers, and home health aides.
  • Attend case conferences, staff meetings, and participate in continuing education as required.
  • Meet all compliance, regulatory, and agency standards for home health nursing.

    • Qualifications:

  • Active RN license in good standing.
  • At least 1 year of recent experience in home health (field experience preferred).
  • Proficient with HCHB documentation software.
  • Demonstrated OASIS expertise.
  • Strong clinical skills and critical thinking abilities.
  • Reliable transportation, a valid driver’s license, and current auto insurance.
  • Excellent organizational and communication skills; comfortable working independently.

    • Preferred:

  • BSN degree.
  • Experience with a weekly caseload of 25–32 points.
  • Familiarity with Medicare and home health compliance.
