An exciting opportunity for an experienced and compassionate Registered Nurse specializing in home health care is available on a 13-week contract basis. In this field-based position, you will deliver skilled nursing services to patients in their homes while advocating for quality, continuity, and individualized care. You will interact directly with patients and families, perform comprehensive assessments, and manage care plans to support optimal health outcomes.

Schedule and Details:

  • Full-time, 13-week contract
  • Monday – Friday, 8:00 AM – 5:00 PM
  • Field-based visits to patient homes (with daily travel and mileage reimbursement)
  • Caseload: 25–32 points per week (roughly 3–5 patient visits daily)
  • Use of Home Care Home Base (HCHB) for documentation and OASIS assessments

    • Key Responsibilities:

  • Complete OASIS assessments and develop individualized plans of care
  • Deliver skilled nursing interventions, including wound care, IV therapy, and chronic disease management
  • Monitor patient progress, identify changes in condition, and collaborate with the interdisciplinary care team
  • Educate patients and their families on clinical care and preventive measures
  • Maintain timely and accurate documentation in HCHB
  • Participate in case conferences, meetings, and ongoing professional development
  • Uphold regulatory and agency documentation and care standards

    • Qualifications:

  • Active RN license in state of practice (in good standing)
  • At least 1 year of recent Home Health RN experience (field experience preferred)
  • Proficiency in using Home Care Home Base (HCHB) and OASIS required
  • Strong clinical assessment, organizational, and communication skills
  • Reliable transportation, driver’s license, and auto insurance
  • Ability to work independently and manage a flexible schedule

    • Preferred:

  • BSN degree
  • Experience managing a 25–32 point-per-week caseload
  • Familiarity with Medicare and home health compliance
