Deliver high-quality, patient-centered care as a Home Health Registered Nurse, supporting individuals in their homes and helping them achieve optimal wellness. This field-based position is suited for compassionate nurses who thrive in autonomous settings and demonstrate a commitment to clinical excellence.

Schedule:

  • Full-time, 13-week contract
  • Monday – Friday, 8:00 AM to 5:00 PM
  • Caseload: 25–32 points per week (approximately 3–5 patient visits daily)
  • Daily travel within assigned territory (mileage reimbursement provided)

    • Key Responsibilities:

  • Conduct thorough OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management.
  • Monitor patient progress, promptly report changes in condition, and coordinate with physicians and the interdisciplinary care team.
  • Educate patients and their families on diagnoses, medications, care plans, and preventive strategies.
  • Document clinical notes accurately and efficiently using Home Care Home Base (HCHB).
  • Collaborate with therapists, medical social workers, and home health aides to ensure comprehensive care.
  • Participate in case conferences, ongoing staff meetings, and continuing education.
  • Stay in compliance with clinical, documentation, and regulatory standards.

    • Qualifications:

  • Active RN license (in good standing)
  • At least 1 year of recent home health RN experience (field experience preferred)
  • Proficient with Home Care Home Base (HCHB) and OASIS assessment tools
  • Demonstrated clinical assessment and critical thinking skills
  • Reliable transportation, valid driver’s license, and appropriate auto insurance
  • Independent, well-organized, and adaptable in a field-based schedule
  • Excellent communication, time management, and organizational abilities
  • BSN and prior experience managing 25–32 points per week preferred
  • Familiarity with Medicare and home health compliance standards preferred
