Step into an impactful advanced practitioner position serving patients within nursing home and assisted living environments. This contract opportunity is tailored for a Nurse Practitioner passionate about providing comprehensive care to insurance members in skilled nursing and assisted living facilities. The focus is on in-place treatment to reduce unnecessary hospitalizations and facilitate successful transitions back home.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • No weekends or holidays required
  • Key Responsibilities:

  • Conduct monthly (or more frequent as needed) health assessments for residents
  • Develop and manage individualized problem lists and medical treatment plans
  • Deliver primary, preventative, urgent, or acute bedside care
  • Stratify high-risk residents and create tailored contingency plans
  • Communicate closely with the residents’ primary care physicians and interdisciplinary teams
  • Collaborate with facility staff, Directors of Nursing, and assigned primary providers to develop and implement care plans
  • Support safe transitions for patients entering or exiting skilled nursing or assisted living facilities, ensuring continuity and quality of care
  • Maintain an assigned panel of patients across multiple facilities, with daily assignments at one location and occasional assessments at additional sites as needed
  • Qualifications:

  • Master of Science in Nursing (MSN)
  • Current and active Nurse Practitioner license in NY
  • Basic Life Support (BLS) certification
  • Experience with nursing home or assisted living patients preferred
  • Strong communication, collaboration, and clinical assessment skills
