Advance your practice in a supportive environment focusing on patient-centered care within nursing home and assisted living settings. As a Nurse Practitioner, you will play a vital role providing comprehensive care to insurance members residing in skilled care and assisted living facilities. The primary mission is to deliver attentive, high-quality care aimed at avoiding unnecessary hospitalizations and promoting successful transitions back to home environments.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • No weekends or holidays required
  • Key Responsibilities:

  • Perform monthly (or more frequent as necessary) health assessments for residents
  • Develop individualized problem lists and tailored medical treatment plans
  • Administer primary, preventive, urgent, and acute care directly on-site
  • Identify and closely monitor high-risk patients, offering increased clinical visits as needed
  • Establish contingency plans for residents with high-risk conditions
  • Collaborate and communicate regularly with the resident’s primary care physician, facility staff, Directors of Nursing, and care teams
  • Coordinate and provide bedside care, create treatment orders, and develop comprehensive care plans
  • Assess patients recently discharged from hospitals, ensuring smooth and healthy transitions out of skilled facilities
  • Manage an assigned panel across multiple nursing facilities, primarily working at one facility daily but occasionally traveling to another for acute needs
  • Qualifications:

  • Master of Science in Nursing (MSN)
  • Active Nurse Practitioner license in New York State
  • Current BLS certification
  • Compensation for this role ranges from $75 to $100 per hour, reflecting experience and qualifications.
