Job Title: Nurse Practitioner (NP)

Location: Jefferson County, NY (Primary SNF locations: Watertown)

Contract Duration: 52 Weeks

Job Description: We are seeking a dedicated Nurse Practitioner (NP) for a 52-week contract in Jefferson County, New York, with primary short-term SNF residents in Watertown. As an NP, your primary responsibility will be to conduct comprehensive assessments, including physical exams, medication reconciliation, medical history, and various assessments to enhance the overall quality of care.

Specific Duties:

  1. Conduct a one-time comprehensive assessment, including physical examination, medication reconciliation, medical history, ADLs, nutritional assessment, cognitive assessment, pain assessment, fall risk assessment, and social determinants of health assessment.
  2. Develop and communicate a problem list and recommended treatment plan for each resident.
  3. Ensure accurate and complete documentation, including ICD10 coding.
  4. Collaborate with the interdisciplinary care team.
  5. Conduct advanced illness and advanced care planning conversations.

Key Program Goals:

  • Improve the overall quality of care for short-term SNF residents.
  • Reduce avoidable hospitalizations.
  • Support residents in developing care plans consistent with their goals.
  • Partner with primary care physicians, residents, responsible parties, and nursing home staff to provide primary/preventive care and personalized care coordination while proactively identifying triggers for acute illness and managing chronic conditions.

    Qualifications:

    1. Active NP license in the state of practice, with no open disciplinary actions, probation, or restrictions.
    2. Minimum of 1 year of NP experience.
    3. Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care.
    4. Reliable transportation to complete assessments in nursing homes, assisted living facilities, and homes.
    5. DEA (Drug Enforcement Administration) certification.

    Schedule: Monday to Friday, 8:00 AM - 5:00 PM
