Job Description:
Are you a dedicated Nurse Practitioner seeking a fulfilling opportunity in the healthcare field? We are currently hiring a Nurse Practitioner for a long-term 52-week contract to conduct comprehensive assessments for patients in short-term SNF stays (90 days or less). The role involves approximately 5-7 visits per day, depending on daily acuity within the resident group.
Specific Duties:
The goal is to enhance overall care quality, minimize avoidable hospitalizations, and support members in developing care plans aligned with their goals. The NP collaborates with primary care physicians, members/responsible parties, and nursing home staff for primary/preventive care and personalized care coordination, while proactively identifying acute illness triggers and managing chronic conditions.

Monday through Friday, 8 am to 5 pm. We are open to discussing options for candidates seeking to work a minimum of 20 hours/week up to 40 hours/week.
