Flexible scheduling allows you to choose your own shifts, with assignments available any day, Monday through Sunday, in areas such as Dunnellon, Ocala, and Belleview.

  • Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage plan members in the community
  • Thoroughly review each member’s prior and current medical history, health conditions, and medication usage
  • Provide patient education related to diagnoses and medication management
  • Identify any existing gaps in care and offer well-informed recommendations
  • Visits typically last 45 minutes and may vary, including annual health or post-discharge assessments
  • No need to prescribe medications or create treatment plans
  • Qualifications:

  • Active Nurse Practitioner license in Florida
  • Board certification through AANP or ANCC required
  • Current BLS (CPR) certification
  • Graduate of an accredited school of nursing, holding an MSN
  • At least 6 months of recent Nurse Practitioner experience is preferred
