Take the next step in your advanced practice career as a Nurse Practitioner specializing in home health risk assessments. Work per diem with flexible scheduling while making a meaningful impact on patient care throughout St. Francois County, Missouri. You will visit Medicare Advantage members in their homes—potentially in Farmington, Park Hills, Bonne Terre, or elsewhere in the region—to complete thorough risk assessments and promote health education.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedule
  • Minimum hourly commitments on a weekly or bi-weekly basis
  • Key Responsibilities:

  • Conduct comprehensive annual health risk assessments for Medicare Advantage plan members
  • Review patients’ medical histories and current health status
  • Provide education on chronic and acute medical conditions, as well as medication usage
  • Identify and document gaps in care; offer recommendations to address these gaps
  • Share valuable information with healthcare partners but do not prescribe medication or create treatment plans
  • Qualifications:

  • Active Nurse Practitioner (NP) license in Missouri
  • Current CPR certification (BLS)
  • Board Certification as a Nurse Practitioner via AANP or ANCC
  • Six months or more of recent NP clinical experience preferred
  • Master of Science in Nursing (MSN) from an accredited nursing program
  • Compensation & Perks:
