  • Key Responsibilities:

  • Conduct detailed annual home health risk assessments for Medicare Advantage members
  • Review and evaluate medical history, current conditions, and medication use
  • Deliver patient education on chronic and acute health issues as well as prescribed medications
  • Identify gaps in care, providing actionable recommendations to ensure high-quality outcomes
  • No treatment plan creation or medication prescribing required
  • Qualifications:

  • Active Nurse Practitioner license in the state of Oklahoma
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited institution
  • At least 6 months of current NP experience strongly preferred
