Deliver essential care and support to the local community by conducting home health risk assessments for Medicare Advantage plan members in Macon County. This per diem opportunity allows Nurse Practitioners to utilize clinical expertise in evaluating patients’ health status, identify care gaps, and provide key education – all in the comfort of patients’ homes. Care visits take place throughout Macon and nearby towns, with flexible scheduling to accommodate your lifestyle.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment
  • Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage plan members
  • Review and evaluate medical history, medical conditions, and medication lists
  • Educate patients about their health and medication management
  • Identify care gaps and recommend solutions to optimize patient outcomes
  • Complete thorough documentation — no treatment planning or prescribing required
  • Required Qualifications:

  • Active Nurse Practitioner license in Missouri
  • Board Certification (AANP or ANCC)
  • Current CPR certification (BLS)
  • MSN degree from an accredited nursing program
  • At least 6 months of current NP experience preferred
