Each assessment is approximately 45 minutes and may include annual health evaluations or post-discharge visits, ensuring meaningful engagement with patients in their own homes.

  • Key Responsibilities:

  • Complete annual health risk assessments for Medicare Advantage plan members
  • Review prior and current medical histories, ongoing conditions, and medication lists
  • Educate patients on managing their health conditions and medications
  • Identify care gaps and make recommendations to support improved outcomes
  • Focus solely on assessments—there is no requirement for prescribing medications or developing treatment plans
  • Qualifications:

  • Active Nurse Practitioner license in Pennsylvania
  • Current CPR (BLS) certification
  • Board certification via AANP or ANCC
  • Minimum of 6 months’ experience practicing as an NP (strongly preferred)
  • Master of Science in Nursing (MSN) from an accredited program
